Temporomandibular Joint (TMJ) Treatment in Chennai
The temporomandibular joint is the hinge just in front of your ear. When it stops working properly, the things you do without thinking — eating, talking, yawning — start to hurt. Jaw pain, clicking, and a mouth that will not open fully are the three complaints we hear most.
Here is the part most patients are relieved to hear: most TMJ problems do not need surgery. We start with the least invasive treatment that will work, and we only move up if it does not. Surgery is the last step on the ladder, not the first.
Your care is led by two oral and maxillofacial surgery (OMFS) consultants, Dr. Pradeep S. and Dr. Kalpa Pandya. The same surgeons see you at the first consultation, plan your treatment, and follow you through recovery. If you want to read more about the scope of this field before you come in, our guide to what maxillofacial surgery covers is a good place to start.
Understanding Temporomandibular Joint Disorder in Chennai
The temporomandibular joint (TMJ) connects your jawbone to your skull. When the joint or the muscles around it become inflamed or damaged, the result is temporomandibular joint disorder (TMD).
It has several possible causes: an injury to the jaw, years of teeth grinding, arthritis, muscle tension from stress, or a jaw that is structurally out of alignment. These are treated differently, so finding out which one you have comes first.
The symptoms range from facial pain that will not settle, to pain on chewing, to a click or pop every time you open, to a jaw that locks. We diagnose with a clinical examination and imaging before we recommend anything. If the clicking and pain are what brought you here, our article on jaw clicking and TMJ pain explains what those sounds usually mean.
Comprehensive Jaw Pain Treatment in Chennai
We treat jaw pain in steps. Conservative, non-surgical care comes first. We only consider surgery if that does not give you relief.
Conservative management: a custom-made nightguard or splint to take pressure off the joint, physical therapy exercises, and medication to settle muscle spasm and inflammation. For a large number of patients, this is where treatment ends.
TMJ arthrocentesis: a minimally invasive procedure in which the joint space is washed out with sterile fluid. This flushes away the inflammatory chemicals and frees up a stiff joint so it moves more smoothly again. It is done as a day procedure.
Surgical intervention: if the joint itself is structurally damaged, TMJ surgery is used to repair or reconstruct it and restore long-term function and comfort. We explain exactly why surgery is on the table before you agree to it.
Lock Jaw and Trismus Treatment in Chennai
Trismus means you cannot open your mouth fully. Patients usually call it lock jaw. It can come from the joint itself, from a dental infection, from an injury, or as a side effect of head and neck cancer treatment. It can also be caused by oral submucous fibrosis (OSMF), a pre-cancerous condition in which the lining of the mouth stiffens and slowly closes it. If your mouth has been getting tighter over months, read why your mouth will not open and the symptoms of OSMF — this one matters, because OSMF is a condition we watch, not just one we treat. It falls under our oral precancer and OPMD service, and where the fibrous bands need to be released surgically, that is an OSMF release.
Because the cause changes the treatment, we work out the cause first. Depending on what we find, that may mean physical therapy, jaw-opening exercises, or medication to relax the chewing muscles. If you woke up unable to open your jaw, locked jaw causes and when to see a surgeon will tell you whether this is urgent.
In TMJ ankylosis, the bones of the joint have fused together after an old injury or a severe infection, and the jaw simply cannot move. Here surgery is not optional. In a TMJ ankylosis release, the fused bone is removed and the joint reconstructed so that mouth opening is restored.
Orthognathic (Jaw Alignment) Surgery in Chennai
Sometimes the joint hurts because the jaw is built out of line. When the problem is skeletal, correcting the alignment is what fixes the pain. Orthognathic surgery reshapes and repositions the upper or lower jaw to correct the bite and the facial proportions.
Dr. Pradeep S. and Dr. Kalpa Pandya plan these operations with orthodontic specialists using digital imaging. Correcting the skeletal imbalance takes strain off the TMJ and improves how you chew and speak. This is one arm of our wider oral and maxillofacial surgery practice, which also covers facial trauma — old jaw fractures that were never set properly are a known route into joint problems.
Expert Care for Patients Across South Asia
The surgeon you choose for a TMJ disorder decides how quickly you get your jaw back. Both Dr. Pradeep S. and Dr. Kalpa Pandya personally handle your diagnosis, your surgical planning, and your rehabilitation afterwards — you are not passed between teams.
We see patients at our partner facility, Apollo Main Hospital on Greams Road. You will be told what we found, what we recommend, and what it will cost, in plain language.
Our clinics are in Chennai, but patients travel to us from across Tamil Nadu, West Bengal, and Bangladesh, and from the wider South India and South Asia regions. Many of our outstation patients speak Bengali or Tamil. If travel is difficult, we offer a remote second opinion so you know what you are coming for before you book a ticket.

What are the common signs that I need TMJ treatment in Chennai?
See a surgeon if your jaw hurts and will not settle, if it clicks or pops when you open, if chewing is difficult, if you get frequent headaches, or if the jaw sticks or locks. A clinical examination tells us the cause. Our guide to jaw clicking and TMJ pain covers what these signs usually point to.
How is a locked jaw or trismus treated?
It depends on why the jaw is locked. If it is muscle tightness or inflammation, we treat with physical therapy, muscle relaxants, and jaw exercises. If the bones of the joint have fused, a TMJ ankylosis release is needed to get the jaw moving again. If the cause is oral submucous fibrosis, treatment runs through the OSMF pathway instead. Read locked jaw causes and when to see a surgeon if this is happening to you now.
What is TMJ arthrocentesis and how does it help?
It is a minimally invasive outpatient procedure. The joint space is gently flushed with sterile fluid, which washes out the inflammatory substances, breaks up minor scar tissue, and lubricates the joint. Pain drops and mouth opening improves. It is one of the treatments covered under TMJ surgery.
Who is a candidate for orthognathic jaw alignment surgery?
People with a severe bite problem, difficulty chewing, long-standing jaw pain, or facial asymmetry caused by a skeletal misalignment that braces alone cannot correct. Our surgeons plan the correction together with your orthodontist.
Is jaw joint treatment covered by health insurance in India?
Many surgical procedures for TMJ disorders — ankylosis release, arthrocentesis, corrective orthognathic surgery — are covered by health insurance policies, especially when they are medically necessary. Our administrative team at Apollo Main Hospital can help with pre-authorisation.
How much does TMJ treatment cost in Chennai?
It depends on what you actually need: conservative care (splints, physiotherapy, medication), a minimally invasive arthrocentesis, or open joint surgery such as an ankylosis release. You get a clear estimate after your evaluation, and most medically necessary TMJ procedures are covered by health insurance.
